It's no longer available at jbunner. It was forked from https://github.com/restena-ma/otrs-saml2sp. Dunno if jbunner do anything more and a wayback machine didn't archived it.

Yep, it seems so.
There was some zombies that didn't died after stop and i didn't checked it second time.
I've killed them, ressurected daemon - now it's working as it should :)

For example - if your dynamic filed name is: MyTestField, then in Dashboard configuration you should add line like: DynamicField_MyTestField. If you added MyTestField - then it's not existing in the ticket...
