I just started working at a company using OTSR. The person who configured it recently left, and he was the only one who knew anything about the system.
The system was up and working for quite some time, but then about a week ago tickets stopped coming in when created by email. Tickets can still be created by the web interface just fine. I have my email client set up to receive emails from the address that is in use by OTRS, so I know the emails are making it to our mail server.
As far as anyone knows, the system has not been modified, updated, or even logged into since the person who set it up left.
The system is running on Ubuntu 9.04 Server (no GUI) and the OTRS version is 2.3.2.
