Ghost agents - invalid agents appearing as options to assign

I'm having a strange problem where invalid agents are appearing at the top of lists of active agents. As an example, in the ticket note window, at the top we have an option to assign the ticket to another agent. At this point the entire scroll window, except for the very bottom line, is filled with blank rows, but each row corresponds to a former employee (invalid agent in the system) and we're losing tickets as they're getting assigned to these "ghost agents." I have attached a screenshot. Anyone ever see this?

what is your OTRS version?

Hello, sorry for the delay in posting back. We are running version 3.0.3.

please update to the latest 3.0.x or better to the latest 3.1.x
