How do I add custom form to OTRS

I would like to add a custom form, capture info and store in database and then generate reports.

I first created a new front end module using Hello World example.
Now I want to add text areas, drop down fields and other form elements.

My question is -
1. Is Dynamic Fields the way to go to add these fields?
2. Or do I add necessary code to DTL file, perl modules, database etc in order to customize the front end module
