Custom Ticket

I want to use OTRS to log tickets and print out the Ticket to use as a Field Service Sheet.
I will use some custom fields for Name, Address etc and have already added them to the NewPhoneTicket settings in Admin.
However - I need some more information on the printed PDF or Emailed Ticket.

I need a disclaimer, "Are you happy with the job" scenario, Customer Signature, JOb Start Time, Job End Time, Any Expenses, Mileage, etc etc....
I'm not sure how to do this or if it's even possible.

I've attached a screenshot of a manual word doc we use for field engineers and need to replicate as a ticket. Styles and layout is not so important but the actual process of space for an engineer to obtain a signature, fill out information etc is.

Any ideas anyone.

OTRS 6.0.x (private/testing/public) on Linux with MySQL database.
