OTRS won't load after fresh installl

Ya I know must be a stupid thing but stuck right now ...
Followed all steps http://otrs.github.io/doc/manual/admin/ ... -otrs.html
Everything seems perfectly set but still OTRS installer.pl, index.pl, customer.pl won't load (Step 8)
Not my first install ... but first one to fail !!! Pride is hurt!!
Apache is running - test page is up
PHP is running - test page is up

perl /opt/otrs/bin/otrs.CheckModules.pl
All Modules are up except for Oracle, PostGresSQL and HANextra are installed
Any Clue??
OTRS 4.0.11
Ubuntu 15.04
MySQL Database, PHP5, Apache (LAMP server bundle)

post the error message

I just get a 404
Not Found
The requested URL /installer.pl was not found on this server.
Apache/2.4.10 (Ubuntu) Server at localhost Port 80
OTRS 4.0.11
Ubuntu 15.04
MySQL Database, PHP5, Apache (LAMP server bundle)

I am guessing this is a rights issue but all seems ok!! grrr..

I ran the otrs.SetPermissions.pl a 3rd time ... for some reason it did not kick in the previous times!!
Now working ...

anyone seen this before?? causes?
