I found the issue. It's something to do with redirects. The fix is to change line 991 of C:\otrs\OTRS\Kernel\Output\HTML\Layout.pm from 6 to 7 seems to fix. I'm having the same issue with version 3.3.6 on Windows Server 2008R2, IIS7 connecting to SQL Server 2008R2.
