I don't know if I solved this the right way, but I solved this issue by editing the default OTRS mail account. I noticed the FROM field when I replied to a customer was set to "otrs@localdomain" instead of my email address. I simply changed this to reflect my email and everything worked ag...

Thanks a lot for pointing in the right direction.
I figured out how to add the custom fields to the ticket form. Is it possible to changed the position of the free text to make appear at the beginning of the form instead of the end after the text box.
