Maybe this file is just a symbolic link? Or web user don't have permission to read from this file?

Probably, there is no login.pl in /opt/otrs/bin/cgi-bin.

If you have queue attached to specific group, then Customer<-->Group Relations should do it as yuri0001 stated.
Set RO to every client and RW with RO for customers which can create ticets for 4th queue.

FreeText fields should be displayed without any changes to template. Maybe they aren't filled?
