checkbox in dtl

I have to develop a web form that contain checkbox,
can you tell me how you do it in otrs?

thank you for replying,
my prblem is I have to show some prerequisites for controle in Request for change,
and it should be checkbox(multiselection),
I tried by adding freetext but I found just type textfield and dropdown,
so I have to develop it,I found how to develop a new web form,thanks to develop manuel
but now I have problem how to add checkbox in my web form

dear tina,
i think you are using otrs lower version.
use the latest version of OTRS, you will get Dynamic field option.

but dynamic field option is not availble for change module,
is just for manage ticket,
