Username Field missing from Customer Registration Page

Hello,

I do not see "Username" field in the customer registration page and also in the add customer user page from the agent interface. How do it get it to show. I would like to have the ability to specify the username instead of autocreation of usernames.

I remember seeing it earlier, and I think something I did that has hidden it from showing. How do it get it back to show. Please help.
