SQL ERROR

We have a user who in any browser cannot create or close ticket. He is an admin in the system but still gets the following error. HELP!

This appears to be related to duplicate login or customer index by which a unique index connection cannot be established. May be a duplicate customer record in the table or null value in index.
OTRS 6.0.x (private/testing/public) on Linux with MySQL database.
