OTRS 4 (Free) version
On the Dashboard, the system reports updates are available up to version 5, but when I go to Admin>OTRS Appliance Manager, the update section says that "Your system is already up to date. There are no updates available."
In the past, I would go here to perform updates. Has that changed or am I missing something here?
Please advise.

The appliance is out of support and there will be no updates for the appliance any more. You have to migrate to a "normal" OTRS installation

As far as I know (and can see @ http://ftp.otrs.org/pub/otrs/) the latest appliance version is 4.0.8.
I suggest to migrate to a standalone system running under a plain Linux

The migration of the data is possible with the CloneDB Addon and or system utilities. You definitely need in-depth knowledge of Linux and OTRS and. If it's critical to your business I suggest to choose professional help on this topic.
