Change Request Issue

Hello Team,

Greetings!!!

There seems be an issue which we are facing while creating Change Requests using ITSM Change Management package. We'll be really grateful if anybody can point us to what is going wrong over here. Brief Description is below:-

OS: CentOS 6.6
OTRS Version: OTRS 4.0.12
Packages:-
GeneralCatalog 4.0.12
ITSMChangeManagement 4.0.12
ITSMCore 4.0.12
WebServer: Apache 2.2
Article location:/opt/otrs/var/article

Issue:-We are using WebUploadCacheModule:FS. We create Change Request in OTRS 4.0.12 with 1 attachment, it works fine.However when we create a Change Request in OTRS 4.0.12 with more than 1 attachment, we get the apache Internal Server error screen. The below screenshot is from the httpd error logs.

There is nothing in the OTRS logs at the time of the issue.

Now when we switch WebUploadCacheModule:DB then it seems to work fine when we create a change request with more than 1 attachment.

For any more information please do let know. Your efforts are really appreciated.
