Communication between 2 OTRS Systems not working.

Does someone knows how the Communication works between two OTRSSystems?

I really have a an Issue with this.
short explain:
One System - New Ticket Ticket#123 - send to 2nd System - new Ticket there with Incident#321
answer from Incident#321 to Ticket#123- but it goes to the last created Ticket in the other System Ticket#125
I don´t have any Postmaster Filters, with Ticketnumberchange. I just have ExternalNumber Recognition for Dynamic Fields.

Sometimes you can see in the Subject, that a "Ghostnumber" (last Ticketnumber) from this System was added at the end.
Like:
IncidentSystem:
mail with Subject: [Incident#321] [Ticket#123] Incident - Customer

IncidentSystem:
follow up with Subject:[Incident#321] Incident - Customer [Ticket#125] - Article will added to Wrong Ticket#125

why does the system add this wrong Ticketnumber and why it deleted the True one and the "Re:"?
