For example by running a email to sms gateway and sending email out from OTRS to this gateway

well, first you need an SMS gateway hardware + sms software
here is the link to a widly used sms software for linux http://smstools3.kekekasvi.com/
after having up and running you can send mail to <a_mobile_number>@<some.internal.domain>
Composing a new reply in OTRS and "To" is the an email addresse like above

Did you have a look at the SMStools http://smstools3.kekekasvi.com/ and furthermore did you try to configure?
Having setup this, we can to further for the mail2sms functionality
