Hi All,
OTRS has been experiencing some weird behavior lately.
Suddenly some activities (like a timeout notification or a customer follow-up) are NEITHER appearing in the log file NOR the History (web-interface) of course, although the agents receive notification emails for those activities.
Also, one customer received an auto-reply with a "WRONG" ticket number, but the Correct email body.
The most recent thing we did is turn on the log rotation (create a log file for every month), could that have a bug?
Can someone please tell me how I can debug this issue?

I guess there is a second system (with a copy of the database) running....

Thanks Jojo, I don't understand how could that be though it sounds so logical
there is one instance of OTRS, and one OTRS database in mysql 
How can I tell if there's something else running that I can't see. This is driving us all crazy, we've run several DB checks to see if it's corrupted and nothing came out, Now we are receiving escalation notifications for closed tickets and several weird stuff.
E.g we got an escalation notification this morning with this subject line:
[Ticket#2012041810000016] Ticket Escalation! ([Ticket#2012041710000036] Coupon problem)
I do appreciate your help and suggestions

check if there is a copy of the system running somewhere... if it is not in history or log it is not sent out by the system

Thank you very much Jojo. There was a "hidden" test system using the same settings (same email and fqdn).
