How to create other fields in customer UI

I wanted to create other field ( other textbox with label) in the customer interface.
My queue is hidden and the SLA
This is what I wanted to do.
Type>
Service>
Gender>
Subject>
Text [ ]

Gender is for example of the field I wanted to create.

Any help? thanks. :)

Any customer field needs to be added/extend schema on the db, or referenced in external backend, then referenced in Config.pm/CustomerUser
OTRS 6.0.x (private/testing/public) on Linux with MySQL database.
