Multiple columns and number boxes in Process ticket

Hi,

Is there any way to get multiple columns in a process ticket? We currently have some forms created with Adobe that we want to get in OTRS but ideally want the formatting to stay the same if possible. See below:

The 2nd question is on number boxes. Is there a way we can do like the above and have a box setup that will only accept a number and error out if not?
