I'm trialing OTRS as a replacement for our in house, homegrown ticketing system, and I'm impressed so far. The feature list is impressive, and the documentation is incredibly comprehensive! I do have a few other questions regarding features and process:
We're an I.T. MSP and we (unfortunately) use Quickbooks for billing our clients for the time we spend working on their issues. Our interface is currently a very kludgy cut, paste, and mark-as-billed from our ticketing system to the invoice window. I'd very much love it if we had a cleaner way to do this (and so would our bookkeeper).
Tracking our time is one of the most important things we do, and not just the billable time. In my explorations, I haven't seen a timesheet type function in OTRS - am I just missing it? Is there some way to allocate time spent on administrative tasks, or even an actual timesheet/ journal function?
As an MSP, we often have clients (Companies, in our current ticketing system) with multiple contacts. We need those employees to be able to see *only their* tickets, and also to be able to designate certain users as supervisors for that site, who *can* see the entire queue for that site.
I'd love a smartphone interface to our ticketing system. Does such a thing exist for OTRS?
For what it's worth, free is not a requirement here; while I love and appreciate (and develop) Open Source Software, I also understand the need to make payroll and other bills, and am happy to talk about paid services or add-ons to OTRS.

There are extension that can be added for timesheet & Iphone.
So you can post your times according to Project & task.

From the iphone App, you can manage the ticket queue (create one, close...). I'm using it and it is very convenient and easy to use.
